Initial Submission

Please use this page to submit QDROs and QDRO related correspondence. All QDROs and QDRO related correspondence must be submitted in PDF format and may not exceed 10 MB or 30 pages. Please ensure the visual quality of the PDF or the submission may be rejected.

If you have more than one QDRO to submit, please complete a separate submission for each QDRO.

If you did not previously download documents via the "Model QDROs and QDRO Procedures" page, you must complete Parts A and B below.

If you did previously download documents, please complete Part A and hit "Search". If a previous record is found, you will then complete an abbreviated Part B. Once all of the required information has been input, upload your documents and/or message.
