Model QDRO and QDRO Procedures

On this page you will find model QDROs and QDRO Procedures to assist you in preparing a QDRO. Prior to filing any documents with the court, parties are strongly encouraged to submit a draft QDRO to UPS for preapproval. You may submit a draft by going to the Home Page and clicking on the "Initial Submission of a QDRO" tab.

Please note that once a model QDRO or QDRO Procedures are downloaded, an administrative hold will be placed on the Participant's account.
